Basic Policy Regarding Protection of Personal Information (Privacy Policy)
The UD Trucks Health Insurance Association takes the following measures to appropriately safeguard information concerning individual members (“personal information” hereinafter).
- The Health Insurance Association implements appropriate safety measures to safeguard the personal information it obtains on its members against leaks, loss, damage, or improper access.
- The Health Insurance Association uses the personal information provided by members solely for purposes considered beneficial for members, such as health maintenance and promotion. In addition, it uses Individual Numbers only within the scope of the purposes specified in the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedure.
- Except when it has obtained advance consent from the member, the Health Insurance Association will not provide personal information on a member to any third party. Additionally, it will not provide personal information containing Individual Numbers (“identifying personal information” hereinafter), whether or not the individual has consented, except in the cases specified in the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedure. However, in line with the Act on the Protection of Personal Information, it may provide personal information on members other than identifying personal information to third parties without obtaining advance consent from members:
- In addition to training and raising awareness among employees regarding personal information protection, the Health Insurance Association will strive to manage personal information appropriately by assigning persons responsible for such management at each section that handles personal information.
- When subcontracting its business operations, the Health Insurance Association will carry out reviews and implement improvements to strengthen personal information protection measures. When concluding business subcontracting agreements, it will carefully examine the competence of subcontractors and consider issues relevant to personal information protection in the content of such agreements.
- A member who wishes to review, revise, or otherwise access his or her personal information may contact the Health Insurance Association's Privacy Contact. The contact there will respond swiftly to such requests to the extent reasonable.
- In addition to complying with laws, regulations, and other standards concerning the handling of members' personal information, the Health Insurance Association continually reviews and strives to improve the content of this Privacy Policy.
Goal of Use of Personal Information
According to the Personal Information Protection Act, when the company handling the personal information (Including our Health Insurance Association) acquires personal information, it must notify or declare the purpose of use of personal information. This Health Insurance Association will declare the objective of using personal information by posting it on Health Insurance bulletin, by publishing it on website, in a newsletter etc.
The main objective of utilizing personal information used by the Health Insurance Association for business purposes are as follows.

- ○Automatic payment of high medical expenses and partial reimbursement
- ○Outsourcing for translation related to overseas medical expenses
- ○Reimbursement to non-life insurance companies for activities related to third party
- ○Joint business of expensive medical care benefits by the Federation of Health Insurance Associations
- Related collection of health insurance premium
- ●Confirmation of insured person's eligibility as well as awareness of standard monthly amount and standard bonus
- ●Collection of health insurance premium
- ●Certification for dependents
- ●Issuance of health insurance card
- ○Outsourcing of data processing for items such as eligibility of insured persons
- Health services
- ●Health maintenance, health checkup for improvement, health consultation and instruction for health maintenance
- ●Management of health improvement facilities (Recreation facilities, etc.)
- ●Implement funding business concerned with high medical expenses and childbirth costs
- ○Consignment to health guidance institutions associated with health guidance and health consultations (Specific health checkups, etc.)
- ○Consignment of medical checkups to medical institutions (Specific medical checkups, comprehensive medical checkup (Ningen dock), colon cancer checkup and stomach cancer checkup etc.)
- ○Operating consignment / joint business of health improvement facilities (Recreation facilities, etc.)
- ○Provision of health checkup results to business operator
- ○Notification of medical expenses to insured person

- ○Consultation or notification to insurance companies, medical institutions, etc. related to compensation work for third-parties

Request for consent to provide personal information to third parties
As per the Personal Information Protection Act, business operators managing the personal information (Including our Health Insurance Association) must not provide personal information to third parties without prior consent of the person. If it is beneficial for insured person or the operator has huge burden then it is not necessary to acquire explicit consent. For things that are not reasonable for insured person however they are announced in advance and insured person has not shown any clear opposition or reservation regarding this then it is considered as “Consent”.
Please contact the Health Insurance Association in writing, in case the consent is not acquired for the below mentioned points. Response will be given as per the Union charter and Personal Information Management protection rules. It will be assumed that consent is acquired in case of absence of request.
- For high medical expenses and additional benefits (Excluding sickness and injured allowance treatment benefits) application for payment is not required. Additionally, payment should be made through employer.
- Based on application, other cash benefits should be paid by employer.
- It is necessary to collect the part of the medical expenses notice pertaining to the households and give it to the insured person.
- For voluntary continuous insured persons, exclude those from 1 to 3 above.
- * Further, please contact in the consultation desk of our Health Insurance Association if consent is not acquired from the dependents members since dependents consent is also required as per the point 3 mentioned above in the medical cost notice.
Regarding shared use of personal information
According to the Personal Information Protection Act, when personal data is shared with a particular person, ① intention of shared use, ② items of personal data to be shared, ③ the shared user, and ④ shared use objectives should be notified or declared in advance.
Our Health Insurance Association will declare the details of joint business by posting on the homepage or the Health Insurance bulletin.
