Marriage
Apply to the Health Insurance Society if the name of the insured person or a dependent has changed or you want to add a new family member as a dependent due to marriage or other reason.
Change of name
If the name of the insured person has changed due to marriage, etc., please notify the business establishment of the name change. Business establishments notify the Health Insurance Society. Once a new health insurance card is prepared, it will be exchanged with the old one.
